People in Tarrant County face many significant barriers to health. In addition, chronic conditions and the factors that lead to them can be more common or severe for certain groups. This seminar is designed to help community-based organizations strengthen their ability to set a fair and adequate baseline of care for all.
Training Objectives: Part II
- Understand the relevance of tracking data on social determinants of health in order to impact local health disparities
- Identify four major components of the collection of data on race, ethnicity, and primary language in racial and ethnic minority populations
- Outline key aspects of a community plan to reduce health disparities among adults with Alzheimer’s and related dementias and other priority populations.
- Develop and evaluate organizational policies/procedures aimed at utilization of data to reduce health disparities.
