Persons with questions regarding available benefits or programs, or who need help accessing benefits may contact a benefits counselor at 817-258-8125.
Issues commonly addressed include Medicare, Medicaid, HMOs, Medicare supplemental policies, long term care insurance, QMB/SLMB, disputes over hospital bills, Food Stamps and other programs people want to access.
Benefits counselors now have the authority to assist with the preparation of advanced directives, including:
- Medical Power of Attorney
- Directives to Physicians
- Do Not Resuscitate (DNR) forms
- Designation of Guardian Before Need Arises
